Gender-Affirming Medical Care Assessments & Letters
In service of our mission to increase access to care, a number of CICA clinicians offer assessment sessions for clients in pursuit of a Letter of Support for Gender-Affirming Medical Care (e.g. Gender Affirming Surgery or Hormone Replacement Therapy [HRT]).
Appointments are available to both new CICA clients, as well as those already working with any of our clinicians.

NEW CLIENTS

Please Note: In order to facilitate this appointment, you’ll be connected with a clinician for a single one-hour session. Clients seeking to continue working with the clinician performing the assessment/writing the Gender-Affirming Medical Care letter must complete the usual client intake process, beginning by filling out this form.
ESTABLISHED CLIENTS
Current CICA clients are encouraged to speak with their clinician about requesting a Gender-Affirming Medical Care letter. Please fill out the form below, and be sure to identify yourself as an existing client. A member of the Intake Team will be in touch to share our clinicians’ remaining availability for this service.
If your clinician is listed above, you may choose to work with them for the assessment, or choose to work with another clinician altogether. Either way, the session designated for this assessment will occur at no cost to you, regardless of any insurance coverage you may/may not have.
